most desktop programs use the system title bar because it just works. but some programs have a custom title bar so that they can change what the title bar shows:
- web browsers replace the window title with tabs
- some text editors add shortcuts like Save/Open/Undo to the left side of the title
- programs like settings and pictures add a Back button to the left side of the title
i think these are good uses for a custom title bar. a bad use of a custom title bar is changing just how it looks or how it's arranged without adding functionality — because the usage can become unintentionally more obtuse for no benefit
